In order to participate in the study, you must register with the Alzheimer’s Association’s essentiALZ® training.
To register, please use the enrollment code provided by the administrator/supervisor.
You will also need your first and last name, your email, and your zip code. All other registration questions are optional.
Registering will take you to:
- Eligibility questions
- A consent form to agree to participate
- An online questionnaire
- The essentiALZ training as applicable
- The essentiALZ certification exam after training
